A-3 After School Enrollment

Welcome to our A-3 School Online Enrollment Page. 

Only one student should be enrolled per form. A new submission for each student you need to enroll is required.  Also once you begin the enrollment form, it does not save until you submit.. Be sure that you have answered all fields with an * and have chosen the correct answer on the yes and no questions. Remember the form does not save and will not let you submit until all required fields are complete.You may type NA in any field which does not apply to your student or household.
